REGISTRATION FORM

    I would like my child to attend the dance classes on a:

    I confirm that as a parent/carer/guardian with the parental responsibility regarding the student named above, I have no concerns that would prohibit their ability taking part in any classes or performances safely.

    I agree that if my child is unwell with temperatures or has an illness that is easily transmitted (covid) they will not attend the dance class until better and negative.

    I agree for any photos/videos of my child to be used for promotional purposes to advertise on social media platform including Facebook /Instagram.

    I agree that my child performing in events, camps and shows can have their names printed in programmes & end of show credits.

    I give permission for my child to have her name written on class attendance registers.

    I give permission for show photos/videos of my child to be uploaded onto our website photo gallery for advertising purpose only. (No names are published)

    I agree to use WhatsApp Group Messaging for communicating class information where each group will be restricted only to Staff and parents/carers of children in those classes and is password protected using end-to-end encryption.

    I understand I have to give half a terms notice in writing before leaving the dance school at any point during the academic year or I will be liable for half a terms fees. This notice requirement applies year-round, and is applied to each academic year throughout their time at VCSOD.

    I understand that once fees are paid these are non-refundable.
